Appointment Information

Neurosurgical Associates can admit and treat patients at the following Lexington Hospitals:


Paperwork and Test Results

It is very important for you to bring any test results or films for testing performed with you on your initial visit.  This includes X-Rays, CT Scans, MRI Scans, EMG's, Bone Scans, and laboratory reports.  The physician will need to review this information in order to provide the best treatment for your condition.

Any forms that we may have sent to you should be completed prior to your appointment.   This will assist us in keeping your waiting time to a minimum.  It is also important that you bring your insurance cards with you so that we can copy them and make sure that we have the correct information for filing your insurance.

Some insurance companies require a referral from your primary care physician.   Some require forms which authorize your visits to us.  For your benefit, please make sure that all paperwork and referral forms are completed prior to your visit.   You may be responsible for the charges for your services should you fail to follow your insurance company's policies and guidelines.


Patient Phone Calls

Staff answering phones. Patient phone calls will be returned by our clinical staff.  We will make every effort to return your call the same day.  If you call is made late in the day, it may be the next day before your call is returned. 

 


 


Physician Assistants

They each have had extensive training and continuing education in order to deliver the best care possible.

 

 

 

 

 




Billing and Insurance

We will file your insurance claim with your insurance company.  You will need to provide us with the insurance billing information so that we may file for services rendered.  Patients are expected to pay any co-pays on the day of service.

You will receive a monthly statement from us for any balance due after your insurance has paid.  We will accept a check, cash, Visa, MasterCard or Discover Card.   If you have any questions about your bill or insurance, please contact our Patient Accounts Representative, who will be happy to assist you.


Participating Providers

We are participating providers with; Staff.

The list above is subject to change without notice.  Please check with your insurance carrier prior to your visit to make sure that we are participating providers with your insurance carrier.

It is not possible for our staff to know all of the requirements of each insurance company.  Please notify us of any special requirements your insurance company may have when you arrive so that we can work together to make sure you receive the benefits you are due from your insurance provider.


Special Insurance Considerations

Auto Accidents - If you have an accident which requires submission of claims to your auto insurance, please provide us with the name and address of the insurance provider, the policy number and the name of the policy holder in addition to your health insurance information.

Workers' Compensation - If your condition is work related, we will need complete information on the insurance carrier covering your employer.  Please provide us with your employer, your employer's workers' compensation insurance carrier and policy information.
